Staying in your own home as you get older shouldn't mean going without support. District 360 delivers Support At Home services under the Australian Government's Support at Home program, working alongside our registered aged care partner, Trilogy Care.

For years we've supported Australians living with disability, building each person's support around their own goals and the life they want to lead. We bring that same approach to older people who want to stay in their own home safely and independently.

This is for anyone aged 65 or over, or 50 or over for Aboriginal and Torres Strait Islander peoples, along with their families and support networks. Here's how the program works, how our partnership fits together, and how to get started.

What is the Support at Home program?

Support at Home is Australian Government funding that helps older people live safely and independently at home for longer. It started on 1 November 2025 and replaces the old Home Care Packages system with a more flexible, rights-based model built around individual choice and control.

The funding covers a broad range of services, from personal care and domestic assistance to allied health, home maintenance and social activities. What you receive is based on your assessed care needs and your personal budget. There are eight funding levels, running from around $11,000 to $78,106 a year, with budgets released each quarter.

The program also brings new rights. The Aged Care Act 2024 introduced Australia's first legislated Statement of Rights, setting out your right to dignity, choice, independence and quality care from compliant, registered providers.

How District 360 and Trilogy Care work together

We work with Trilogy Care, and each of us has a clear role. It's worth understanding the split, because it's what makes this different from a standard provider arrangement.

District 360 delivers your care. We're the ones who come to your home. We plan, coordinate and deliver your in-home services, including personal support, domestic assistance, meal preparation, social outings and more. You see a consistent team, and your care is built around you.

Trilogy Care manages the funding. Trilogy Care is a registered aged care provider and an approved Support at Home funding provider. They handle the financial administration of your government funding for a low, transparent fee, which means more of your budget goes to actual care hours with District 360.

Because Trilogy Care keeps its fee low, your budget stretches further and you get more hours of quality support from us.
