About the Role

As a Service Facilitator at District 360, you will play a crucial role in ensuring the seamless delivery of services to our participants. Your primary responsibilities will include coordinating with various departments to meet service requirements, managing participant relationships, and ensuring that all services are provided in a timely and efficient manner. You will work closely with the Service Delivery Manager to identify areas for process improvement and help implement new strategies that enhance service efficiency. The ideal candidate will possess strong organisational skills, excellent communication abilities, and a customer-focused mindset. This role offers an opportunity to directly impact client satisfaction and contribute to the overall success of the organisation.

Requirements

Key Responsibilities


  • Participant Engagement: Work with participants and their support team to achieve their goals in line with their NDIS plan. Guide the support work team in breaking down these goals.

  • Coaching: Use a coaching approach to assist participants in capacity building and guide the team of support workers.

  • Case Management: Maintain accurate and up-to-date case notes. Coach the support work team to do the same and adhere to the company's processes and procedures.

  • Safety Compliance: Ensure Work Health and Safety guidelines are adhered to.

  • Stakeholder Collaboration: Engage with participants and work collaboratively with their stakeholders, families, and any other formal and informal supports.

  • Auditing: Undertake regular internal audits to ensure all supports in place align with the participants' funding in their NDIS plan.

  • Continuous Improvement: Contribute to continuous improvement and provide feedback for business growth and development.

  • Policy Adherence: Undertake work in line with the organisation's policies, procedures, and values.

  • Legislative Compliance: Comply with all relevant legislation and code of conduct.

  • Knowledge Maintenance: Maintain an up-to-date knowledge of the NDIS system and practices.

  • Professional Boundaries: Maintain clear professional boundaries with participants.


Qualifications


Required: 

  • Hold a current Driver's License.

  • A roadworthy vehicle with comprehensive insurance.

  • Hold a current Working with Children Check for paid employment.

  • NDIS Worker Screening Check.

  • Australian or New Zealand resident.

  • Completion of District 360 Prerequisite trainings (online L&D access will be provided to be completed prior to joining)

Preferred: 

  • Certificate IV or above in a relevant field (desired) or equivalent work experience.
