About the Role
District 360 is seeking a dedicated and compassionate Psychosocial Recovery Coach to join our team. This casual role involves working collaboratively to design, plan, implement, and monitor recovery plans for participants with psychosocial disabilities. You will support participants by emphasizing their strengths, resilience, and skills while helping them discover various services and supports to maintain a robust support network.
Requirements
Psychosocial Recovery Coaching
Design and Implement Recovery Plans: Work collaboratively with participants to design, plan, implement, and monitor individualized recovery plans aligned with their NDIS goals and aspirations.
Strengths-Based Coaching: Encourage and support participants by focusing on their strengths, resilience, and skills.
Service Navigation: Assist participants in discovering and navigating different services and supports to build and maintain a strong support network.
Principles of Recovery: Ensure that all support provided reflects the principles of person-centered care, psychosocial disability and recovery, trauma-informed practice, supported decision-making, and family-inclusive practice.
Administrative Duties: Handle administrative tasks related to participant management, including service requests, referrals, progress reports, and preparation for NDIS Plan Review processes.
Relationship Management
Stakeholder Engagement: Develop and maintain effective working relationships with mental health sector stakeholders to ensure accountability and build service pathways for NDIS participants.
Cultural Competency: Work effectively with individuals from Indigenous and Culturally and Linguistically Diverse (CALD) backgrounds.
Uphold Values: Reflect TeamHEALTH's values of Integrity, Accountability, Wellbeing, and Respect in all engagements.
Reporting and Evaluation
Progress Reporting: Prepare regular NDIS progress reports, incorporating all aspects of the participant's life.
Expenditure Tracking: Monitor individual NDIS plan expenditures.
Case Notes: Ensure participant case notes are updated daily.
Policy Development: Collaborate with the Team Leader to develop relevant policies, procedures, tools, and operational guidelines.
Professional Development: Engage in regular support and supervision, continued professional development, and team meetings.
