About the Role

As a Support Coordinator, you will play an integral role in supporting participants to achieve their goals in accordance with their NDIS plans. This includes providing ongoing support coordination services, engaging with participants, and collaborating with their supports and third-party organizations in a professional manner.

Salary: 45.13 - 48.58

Requirements

We are looking for an experienced and passionate Support Coordinator to join our Perth team. In this role, you will work alongside NDIS participants to help them understand and implement their plans, connect with appropriate supports, and build their capacity to navigate the NDIS with growing confidence.

You will manage a caseload of participants across a range of disability types and support needs, working collaboratively with participants, their families, allied health professionals, SIL providers, and the broader service system.

Key Responsibilities:

  • Implement and coordinate supports in line with participant NDIS plans and goals

  • Develop and maintain Support Coordination Plans, Progress Reports, and Plan Review reports within required timeframes

  • Connect participants with appropriate funded and community supports, including allied health, SIL arrangements, and informal networks

  • Build participant and carer capacity to self-manage and navigate the NDIS over time

  • Liaise with NDIA planners, Local Area Coordinators, and other stakeholders on behalf of participants

  • Monitor participant progress and escalate complex issues or risks in a timely manner

  • Maintain accurate and compliant records in line with NDIS Practice Standards and D360 internal requirements

  • Ensure all service bookings and claim submissions are completed accurately and on time

  • Contribute to team meetings, supervision, and continuous improvement activities


About You

To be successful in this role, you will bring:

 

  • 1 to 2 years of experience in a Support Coordinator or similar NDIS coordination role (preferred)

  • Sound knowledge of the NDIS, including plan funding categories, support budgets, and the MyPlace Provider Portal

  • Proven ability to manage a mixed caseload with competing priorities and deadlines

  • Strong written and verbal communication skills, with experience producing high-quality reports for plan reviews and progress notes

  • A genuine commitment to person-centred practice and participant empowerment

  • Confidence working independently in the community while remaining connected to your team

  • Current NDIS Worker Screening Check (or willingness to obtain)

  • Current Working with Children Check (or willingness to obtain)

  • Valid driver's licence and access to a reliable vehicle

  • First Aid and CPR certification (or willingness to obtain)


Desirable

  • Tertiary qualification in Social Work, Disability Studies, Community Services, or a related field

  • Experience supporting participants with complex psychosocial, ABI, or high physical support needs

  • Familiarity with Specialist Support Coordination and complex NDIS plan management

  • Bilingual language skills (Perth community languages highly regarded)
