Not necessarily. Get in touch and we'll talk you through the steps to access an assessment.

  • Can I keep my existing care workers?

Absolutely. Self-management gives you the freedom to choose your own providers — including keeping the District 360 team you already know and trust.

  • Is District 360 a registered aged care provider?

District 360 is your care provider — we deliver the support. Trilogy Care is the registered approved provider who administers your Support at Home funding from the government. Together, we've got you covered.

​

  • How does billing work?

We handle it. District 360 submits invoices directly to Trilogy Care for payment from your funding. No paperwork, no stress.

​

  • ​What's the difference between Self Managed and Fully Coordinated?

Self Managed means you're in the driver's seat — you decide who supports you and how your budget is spent. Fully Coordinated means a Trilogy Care coordinator handles the logistics for you (and they'll recommend District 360 as your first choice).
